Patient Access to their record

Patients can view and print their records online i.e. appointment bookings, medications, allergies and adverse reactions.  If you are not registered for the online system and wish to do so please come into the surgery with photographic ID (eg. passport or driving licence) and proof of address (bank statement or council tax bill) and a receptionist will assist you with the process.

As from 1 April 2016

Patients now also have the option to request further access to their record which includes more detailed clinical notes  If you wish to request this please go into "Patient Record" in your online account and click on the appropriate button to make the request.

You will also need to complete a registration form for this additional service, please print and complete the form below (also available from reception) and hand into the surgery, this will be available within 90 days. We have also attached our protocol for online services and a helpful patient leaflet.
